I hereby consent to Mentor Canada and its founding partners (Big Brothers Big Sisters of Canada, The Alberta Mentoring Partnership and The Ontario Mentoring Coalition) the use of any photographs, audio and/or video recordings of myself as taken or produced by media personnel and/or Mentor Canada or Local Partner staff at events or otherwise authorized by Mentor Canada and that this media may be used by Mentor Canada and its founding partners for purposes of promotional material including brochures, posters, newsletters, media information, advertisements, audio-visual productions and digital media(such as websites and social media). Photographs or video productions may also be shared with partners for promotion. I further waive any claim which I may have against Mentor Canada, its founding partners and its fiscal agent Big Brothers Big Sisters of Canada, arising from the use of such photographs, audio and/or video recordings of myself, as aforesaid. This consent and waiver shall remain in effect unless otherwise revoked.

I consent to receive commercial electronic messages such as emails, newsletters, promotional and informational messages from or sent on behalf of Mentor Canada, the Canadian Mentoring Partnership, an initiative of Big Brothers Big Sisters Canada about its programs and services, and in relation to online mentor training and other mentoring programs.
